A Multi-disciplinary Team Working Towards Your Rehab Goal

Once your individualised rehab programme is set in motion, your rehab team gets into action with you. Your condition is assessed by the rehabilitation physician, therapists and psychologists and will continue to be monitored throughout. Team conferences are held at least weekly to review your progress, celebrate your achievements and set new rehab goals with you.

Becoming Part Of Your Extended Family

Family plays a significant role in your recovery outcomes, and holistic care should extend to support caregivers and loved ones as well. Our rehab team also journeys with the family during this season, offering transitional care training (commonly referred to as carer training) and encouraging their participation from the early stages of the rehab programme.

Rehab Nurses: Care Coordinators, Advocates, Change Agents

Rehab nursing takes on a different approach from acute and regular nursing care. Rehab nurses are sub-specialised to cater to your daily rehab needs. They are the binding agent throughout the rehab programme, carrying out and reinforcing what you’ve learnt so that it sticks until you achieve your rehab goals. The nursing team encompassess multiple roles to make your inpatient stay a comfortable one.

Our team of therapists includes physiotherapy, occupational therapy and speech & language pathology.

Patient Centred Healthcare

You & Your Family

Yourself, as the patient, together with the love & support of your family are the central focus of the rehab team. The rehab team's decisions and programme design revolves around your needs and personal goals.

Physiotherapist

Therapists who help to restore functions related to movement, muscle strength, exercise, and joint function. Our physiotherapists are sub-certified in areas including neurorehabilitation, musculoskeletal, sports rehabilitation and performance, cardiac rehabilitation, Parkinson's programme, amongst others.

Occupational Therapist

A key role in restoring independence through functions related to activities of daily living (ADLs) including work, school, family, and community and even leisure activities. Occupational therapy begins in inpatient rehabilitation and is also available as outpatient.

Speech Therapist

Speech therapists help to assess, treat and restore functions related to speech, language, cognitive, social and cognitive communication, or swallowing issues in adults and children.

Dietitian & Nutritionist

They evaluate and provides for your dietary needs depending on the medical needs, eating abilities, and food preferences. Our nutritionist also serves the wellness & integrative medicine stream through managing dietary lifestyle habits.

Physicians

There will be a lead physician who evaluates, prescribes and manages your rehab programme with the support of the rest of the rehab team. They chair weekly team conferences to review each patient's progress. Visiting physicians such as neurologists, orthoapedics, cardiologists, geriatricians, sports physicians, are available to co-manage your rehab programme.

Rehab Nursing Team

Nurses who specializes in rehabilitative care and assist you in achieving maximum independence in activities of daily living (through indirect therapy). This is a very different approach from acute nursing. Their focus is on round-the-clock medical care, prevention and management of complications, and patient & family education.

Case Coordinator

A nurse counselor who acts as a liaison for the patient, family, and rehab team from the first contact point. They conduct preliminary rehab assessments, helps provide support and coordinate discharge planning and referrals.

Pharmacist

Medication management and administration support during the rehabilitation programme is important to ensure you are feeling well to participate in therapy.

Psychologist/Psychiatrist

A healthcare provider or counselor who conducts cognitive assessments and may help the patient and family adjust to any new lifestyle modifications post-incident. Neuropsychologists are sub-specialized in understanding the relationship between the physical brain and behaviour, including conditions or disorders within the brain and nervous system which can alter behaviour and cognitive function.
